Download our forms, documents, and reports. Note that all Plan documents are PDFs and are best viewed on devices with a large screen, such as personal computer or tablet device.

For any of our fillable forms we suggest the following steps:

1. Download the form to your computer
2. Fill it out and save the form
3. Print it out
4. Sign the form (legal requires a physical signature)
5. Scan in the final form and email, fax, or upload the form on the IAFF MERP Member Portal
